When to Call 911 (Emergency Warning Statement)
Do NOT use All Concierge Medical services, including telehealth, for life-threatening emergencies.
Call 911 immediately or go to the nearest emergency department if you are experiencing:
Chest pain or pressure
Difficulty breathing or shortness of breath
Severe abdominal pain
Sudden weakness, numbness, or trouble speaking
Loss of consciousness or fainting
Severe bleeding that does not stop
Head injuries or major trauma
Signs of a stroke (facial drooping, arm weakness, slurred speech)
Suicidal thoughts or intent to harm yourself or others
Severe allergic reaction (difficulty breathing, swelling of lips/tongue)

When to Seek Urgent or Emergency Medical Attention
If you experience any of the symptoms below, you should seek urgent care or emergency evaluation, even if you have an appointment scheduled with us:
High fever not responding to medication
Persistent vomiting or dehydration
Severe or worsening infection
New or severe headaches
Injuries requiring X-ray for suspected fractures
Severe back or neck pain
Worsening or uncontrolled symptoms of chronic illness
Telehealth and concierge services are not a substitute for emergency evaluation.

All Concierge Medical complies with all applicable HIPAA privacy and security regulations.
Your personal health information (PHI) is protected and will only be used or disclosed as permitted by law.
Electronic communications, including telehealth, are encrypted whenever possible.
Patients are encouraged to ensure their own privacy by using secure devices and participating in telehealth visits in a private setting.
By using our telehealth or electronic messaging services, you acknowledge that no platform can guarantee 100% security, though all reasonable safeguards are in place.

Unaccompanied Minors Policy
To ensure safety, legal compliance, and appropriate care:
Minors (under 18 years old)
Must be accompanied by a parent or legal guardian for most medical visits.
A parent/guardian must provide consent for treatment, telehealth services, labs, testing, and medications.
Exceptions (when allowed by state law):
Minors may seek care without a guardian present for:
STI screening/treatment
Pregnancy-related care
Mental health evaluations (state-specific)
Substance abuse evaluation
(Varies by jurisdiction; clinic will follow applicable laws.)
A signed consent-to-treat-minor document may allow some services when the guardian cannot physically attend.
💬 Telehealth Limitations Disclaimer
Telehealth visits provide convenient access to medical care; however, they have limitations:
Providers may be unable to perform a complete physical examination.
Some conditions may require in-person evaluation for accurate diagnosis or treatment.
Telehealth cannot be used for emergencies or severe symptoms.
If your provider determines an in-person visit is necessary, you will be advised to seek immediate care.
Medication & Prescription Disclaimer
Prescriptions are provided based on clinical judgment and are not guaranteed.
Certain medications, including controlled substances, weight-loss medications, and psychiatric medications, require strict guidelines and may require periodic in-person evaluation.
Patients must follow up as instructed to continue receiving prescriptions safely.
